Preparing you for a productive visit
In order to get the most out of your visit, it helps for you to be prepared. At your initial visit, we will ask detailed questions regarding your symptoms, conduct a physical exam, and review relevant imaging results. By providing details of your symptoms and previous tests (such as CT, MRI, Angiogram) and any treatments (physical therapy, epidural steroid injection, previous surgery), we will better be able to advise you. It is crucial to have access to view relevant imaging (rather than reports only); you may be asked to obtain copies of imaging prior to your appointment.
Scheduling Your Appointment
When calling to schedule an appointment, please have the following information available:
- Calendar
- Name of referring physician
- Information about recent tests/treatments
- Insurance Card/Cards

Our care for our patient continues after your surgery has taken place. Below please find some valuable resources for your followup care.
Forms
In order to communicate with other doctors and to best understand the nature of your current symptoms your and previous medical history, we ask that you please complete the New Patient Packet and bring it with you to your first appointment.
